Welcome to LMS! My goal is to care for your child from a health perspective in the best possible way during the day when he/she is at school. Please take a moment to read over the policies below to make sure that your child's health needs may be addressed here at school. Immunization Records - Kansas State law requires that all students attending LMS have documentation of immunization records on file in the nurses office. Please make sure you try and get your childs most recent records to my office and that will help ensure that you don't hear from me later in the year that your child is missing an immunization. Most of the time, the child has already had the immunization, but I just don't have the most recent copy for my records. Please be aware that for the 2012-2013 school year, all 7th and 8th graders will need to have 2 doses of Varicella (chickenpox) and a single dose of Tdap is required at grades 7-10. | ||

Health Screenings - Vision and Hearing screening will be administered to all 7th grade students as well as new students in the district. Screenings may also be administered at parent request. These screenings will take place in September and we will send the date via parent email as it gets closer. Medications - the school nurse must give all medications required by students during the school day. Prescription medication must be in the original container. In addition, the medication must be accompanied by a note from the physician requesting that the medication be given at school. Over-the-counter medication may be dispensed with parental permission on file in the nurse's office.
